- Conduct thorough assessments of residents' needs and develop tailored care plans
- Monitor and document changes in residents' health status
- Collaborate with the Resident Care Director to ensure effective implementation of care plans
- Provide direct care and support to residents as needed
- Maintain accurate records and reports
Qualifications:
- Degree in Nursing (RN) from an accredited institution
- Current state license as a Registered Nurse
- Minimum one year experience in home health, assisted living, hospital, or long-term care environment
- Strong knowledge of nursing practices, techniques, and methods
- Excellent communication and interpersonal skills
- Able to work effectively in a fast-paced environment
