Job Responsibilities
As a Wellness Nurse at Sunrise Senior Living, you will be responsible for:
- Providing high-quality resident services, including conducting monthly wellness visits and monitoring fall risk
- Working closely with the RCD to identify ongoing needs and ensure resident satisfaction
- Maintaining accurate records and documentation in resident wellness files
- Communicating effectively with resident families and responsible parties
- Collaborating with the healthcare team to ensure seamless care delivery
Qualifications
To succeed in this role, you will need:
- A graduate degree from an approved college/school of nursing
- A current state license as a professional LPN/LVN
- Experience in home health, assisted living, hospital, or long-term care environments
- Strong organizational and time management skills
