Responsibilities
- Identify ongoing needs and services of residents to promote high-quality resident services
- Ensure proper follow-through and participate in evaluation and documentation for residents with changes in healthcare services and status
- Assist Resident Care Director (RCD) in conducting resident evaluations and gathering data at move-in, 30 days post-move-in, semi-annually, and with any change in status
- Conduct monthly wellness visits for all residents
- Monitor and follow up with residents who have experienced falls, accidents, or incidents; document and report as required and directed
Qualifications
- Graduate of an approved college/school of nursing
- Current state license as a professional Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN)
- Minimum one year experience in home health, assisted living, hospital, or long-term care environment
