Job Description:
Sunrise Senior Living is seeking a skilled LPN to join our team as a Medication Care Manager. In this role, you will be responsible for providing high-quality care and services by administering medications and treatments in a safe and organized manner.
Responsibilities:
- Administer medications and treatments to residents according to their individual needs and medical plans
- Conduct regular medication reviews and updates with the Resident Care Director (RCD) or Wellness Nurse
- Maintain accurate and up-to-date records of medication administration, including documentation of any refusals or missed doses
- Participate in daily Cross Over meetings and contribute to the development of Individualized Service Plans (ISPs)
- Collaborate with other healthcare professionals to ensure seamless care delivery and resident satisfaction
Qualifications:
- Licensed Practical Nurse (LPN) or state-specific equivalent
- CPR and First Aid certification
- Ability to work independently and as part of a team
- Excellent communication and interpersonal skills
- Flexibility to work varied shifts, including weekends and holidays
