We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team as a Healthcare Services Coordinator. In this role, you will be responsible for coordinating the delivery of healthcare services to our residents, ensuring their medical needs are met.
Responsibilities:
- Coordinate Resident Care: You will coordinate the delivery of care to residents, including medication administration, vital sign monitoring, and wound care.
- Communicate with Residents and Families: You will communicate effectively with residents and their families, keeping them informed about their care and responding to their concerns.
- Maintain Accurate Records: You will maintain accurate and up-to-date records of resident assessments, care plans, and progress notes.
- Work Collaboratively with Team Members: You will work collaboratively with other team members, including physicians, therapists, and social workers, to ensure comprehensive and coordinated care.
Qualifications:
- Graduate of an approved college/school of nursing
- Current state license as a professional Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN)
- Minimum of one year experience in home health, assisted living, hospital, or long-term care environment
