Job Summary
As a Wellness Nurse at Sunrise Senior Living, you will play a critical role in ensuring the overall well-being of our residents. You will identify ongoing needs and services, conduct monthly wellness visits, and provide clinical support to our team members.
Key Responsibilities
- Identify ongoing needs and services of residents to promote high-quality resident services
- Conduct monthly wellness visits for all residents
- Monitor and follow up with residents who have experienced falls, accidents, or incidents; document and report as required and directed
- Participate in evaluation and documentation for residents with changes in healthcare services and status
Requirements
- Graduate of an approved college/school of nursing
- Current state license as a professional Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN)
- Minimum one year experience in home health, assisted living, hospital, or long-term care environment
