Job Description
As a Wellness Nurse at Sunrise Senior Living, you will play a vital role in ensuring the overall well-being of our residents. Your responsibilities will include:
- Conducting monthly wellness visits and monitoring fall risk
- Working closely with the RCD to identify ongoing needs and ensure resident satisfaction
- Maintaining accurate records and documentation in resident wellness files
- Communicating effectively with resident families and responsible parties
Required Skills and Qualifications
To be successful in this role, you will possess:
- A graduate degree from an approved college/school of nursing
- A current state license as a professional LPN/LVN
- Experience in home health, assisted living, hospital, or long-term care environments
- Strong clinical skills and knowledge of federal, state, and local regulations
