About the Role
We are seeking a dedicated and compassionate LPN Medication Care Manager to join our team. As a key member of our healthcare staff, you will play a vital role in ensuring the highest quality of care and services for our residents.
- Administer medications and treatments in a safe and organized manner
- Document medication administration and resident interactions
- Conduct Service and Health Updates as directed by the Resident Care Director
- Participate in the development and implementation of Individualized Service Plans (ISPs)
Responsibilities
- Medication Administration:
- Review and initialize Daily Logs to document resident information
- Administer and assist with medications and treatments
- Ensure accurate documentation of medication administration
- Resident Care:
- Notify the Resident Care Director of any resident or family concerns
- Attend daily Cross Over meetings to discuss resident care plans
- Conduct Service and Health Updates as directed
- Risk Management and General Safety:
- Report all accidents and incidents immediately
- Follow safety procedures and protocols
- Ensure compliance with OSHA and other regulatory requirements
Requirements
- Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) credential
- CPR and First Aid certification
- Ability to make choices and decisions in the best interest of residents
- Excellent communication and problem-solving skills
