Job Overview
The Care Manager at Sunrise Senior Living is responsible for delivering personalized care and services to a consistent group of residents and their families in our assisted living and reminiscence neighborhoods. This role requires building trust, communicating effectively, and working collaboratively as part of a dynamic team.
Responsibilities and Qualifications
- Build meaningful relationships with seniors and their families, providing assistance with activities of daily living, attending to individual care needs, and getting to know their unique preferences and personalities.
- Notify management of changes in condition and recommend adjustments in the level of care and service.
- Assist residents in life skills and other life-enriching activities as indicated on their individual profile.
- Participate in the development of Individualized Service Plans (ISPs) and monthly updates.
- Be responsible for designated residents during shifts, knowing their whereabouts and physically checking on them throughout the shift.
- Partner with the community team to ensure compliance with OSHA requirements and promote Risk Management programs and policies.
- Respond to dining needs of residents and guests while maintaining Sunrise hospitality and service standards.
- Invite, Encourage, and Assist (IEA) residents in life skills and other life-enriching activities as indicated on ISP and demographic profiles.
Required Skills and Qualifications
- Dedication to serving seniors with excellent customer service skills.
- A positive attitude, flexibility to perform various duties, and the ability to work in a team environment are key to success.
- A High School diploma or GED is accepted and may be required per state regulations.
- In states where applicable, must maintain certifications.
- Must be at least 18 years old.
- Previous experience working with seniors is preferred.
- Ability to make choices, decisions, and act in the resident's best interest.
- Possess written and verbal communication skills and a level of understanding.
- Competent in organizational and time management skills.
- Demonstrate good judgment, problem-solving, and decision-making skills.
