Job Overview

The Care Manager at Sunrise Senior Living is responsible for delivering personalized care and services to a consistent group of residents and their families in our assisted living and reminiscence neighborhoods. This role requires building trust, communicating effectively, and working collaboratively as part of a dynamic team.

Responsibilities and Qualifications

  1. Build meaningful relationships with seniors and their families, providing assistance with activities of daily living, attending to individual care needs, and getting to know their unique preferences and personalities.
  2. Notify management of changes in condition and recommend adjustments in the level of care and service.
  3. Assist residents in life skills and other life-enriching activities as indicated on their individual profile.
  4. Participate in the development of Individualized Service Plans (ISPs) and monthly updates.
  5. Be responsible for designated residents during shifts, knowing their whereabouts and physically checking on them throughout the shift.
  6. Partner with the community team to ensure compliance with OSHA requirements and promote Risk Management programs and policies.
  7. Respond to dining needs of residents and guests while maintaining Sunrise hospitality and service standards.
  8. Invite, Encourage, and Assist (IEA) residents in life skills and other life-enriching activities as indicated on ISP and demographic profiles.

Required Skills and Qualifications

  • Dedication to serving seniors with excellent customer service skills.
  • A positive attitude, flexibility to perform various duties, and the ability to work in a team environment are key to success.
  • A High School diploma or GED is accepted and may be required per state regulations.
  • In states where applicable, must maintain certifications.
  • Must be at least 18 years old.
  • Previous experience working with seniors is preferred.
  • Ability to make choices, decisions, and act in the resident's best interest.
  • Possess written and verbal communication skills and a level of understanding.
  • Competent in organizational and time management skills.
  • Demonstrate good judgment, problem-solving, and decision-making skills.
