Job Description

• Assist Resident Care Directors in conducting resident assessments and ensure proper follow-through on healthcare services and status changes.
• Conduct monthly wellness visits for all residents, assessing their health and well-being, and document pertinent information in resident wellness files.
• Maintain open communication with residents' families and responsible parties to ensure medical needs are met, and collaborate with attending physicians when necessary.
• Ensure weights and vital signs are obtained monthly for each resident, and assist in maintaining wellness files according to Sunrise policies and federal regulations.
