Responsibilities
- Implements all consumer Individual Service Plans
- Prepares screening, intake and orientation of potential and new residents
- Performs Case Management services
- Attends all appropriate inter-agency meetings
- Completes chart reviews as required
- Develops and provides on-going staff training
- Ensures agency compliance with all Federal, State and local regulations
- Possess a genuine interest in and concern for the consumers
- Provides continuous training for residents and staff regarding mental health and residents’ rights issues
- Pursues continuing education for professional growth
- Perform all other responsibilities as assigned

- Two years’ related work experience in a mental health capacity with case management duties
