What are the responsibilities and job description for the Glycemic Control Nurse Practitioner position at SUNY Upstate Medical University?
Job Summary:
The Glycemic Management Nurse Practitioner will serve as a full time Advanced Practice Provider to provide care to patients at SUNY Upstate Downtown and Community Campus. The APP will provide Inpatient Glycemic Management of the diabetic patient.
Key Responsibilities include:
Providing expert level clinical care & coordination of care via a full range of services that will improve quality of patient care.
Assessing & evaluating patients.
Ordering appropriate laboratory testing & diagnostic studies.
Arranging for referrals, testing, consultation & therapeutic services.
Maintaining outstanding relationships with families, referring physicians & all members of the health care team.
As an SUNY Upstate APP, you will be an integral member of the multidisciplinary healthcare team, demonstrating professionalism, leadership, and a commitment to Upstate’s mission, vision, and values, while adhering to all institutional policies and procedures.
Minimum Qualifications:
Master’s Degree as a Nurse Practitioner.
NYS Licensed/Registered NP.
Current National Certification consistent to the clinical practice.
NYS licensed/registered NP or PA required. Have a NPI number and be Medicare enrolled with a Medicare Identification Number (or PTAN) (or eligible for Medicare Enrollment within 6 months of hire.
CPR-BLS Certification required at the time of appointment.
Must successfully pass pre-employment and credentialing clearances.
Preferred Qualifications:
Endocrine or Glycemic management experience preferred.
Work Days:
Monday - Friday, Days with possible weekend rotation.
