Job Summary :
The Glycemic Management Advanced Practice Provider will serve as a full time Advanced Practice Provider to provide care to patients at SUNY Upstate Downtown and Community Campus. The Advance Practice Provider will provide inpatient glycemic management of the diabetic patient. Responsibilities include providing expert level clinical care & coordination of care via a full range of services that will improve quality of patient care assessing & evaluating patients, ordering appropriate laboratory testing & diagnostic studies, arranging for referrals, testing, consultation & therapeutic services, and maintaining outstanding relationships with families, referring physicians & all members of the health care team.
Minimum Qualifications :
NYS licensed / registered NP or PA required. Master's Degree as a Nurse Practitioner or as a Physician Assistant and current National Certification consistent to the clinical practice also required. CPR-BLS Certification required at the time of appointment.
Preferred Qualifications : Inpatient, Endocrine or Glycemic management experience preferred Work days :
Monday - Friday, Days with possible weekend rotation.
