Summary : Creighton University Medical Center and University Campus (stand alone) are in need of locum coverage.

RATE CAPS FOR SUBMISSION :
Malpractice Must be included in rates.
Orientation half the hourly / callback rate kicks in after 8 hours. Per hour of orientation or training beyond 8 hours
Hourly xx300 and below Per hour not OT
Holiday Must be 1.5x the regular rate. Per holiday hour no OT

- 6 : 30am3 : 30pm
- 11 : 00am8 : 00pm
- 3 : 30pm12 : 30am

Facility Location : CUMC and University Campus
Setting : ER

Duties : Routine ER duties; codes; ER locums dont have a lot of trauma responsibilities

Credentialing Information :
Timeframe : 60 days (completed application needs to be returned to facility 48 hours after receipt)
Requirements : Case logs from facilities need to be returned ASAP;
Minimum Education : M.D. D.O.
Training : Completion of an ACGME / AOA approved Emergency Medicine residency program.
Experience : Actively practicing as an Emergency Medicine Physician for the last 12 months or completion of an Emergency
Medicine Residency in the last 6 months
Credentialing with other facilities University Campus
Credentialing Deadlines are listed in attached document. Please ensure when submitting a candidate that these deadlines can be met.
Billing enrollment Is this position providing services in lieu of an existing enrolled provider for a period of 60 days or less : No
