Support, Inc. is primarily a child/adolescent mental health and substance use provider serving Gaston, Cleveland, Lincoln, and other surrounding counties for over 20 years. Support, Inc. provides the full array of child/adolescent mental health and substance abuse services, with the exception of level 3 and PRTF services.
Support Incorporated is seeking a case manager. Qualified Professional (related bachelor's degree with two years of experience with the child and adolescent mental health population or a bachelor's degree that is not related and four years of experience with the population served) credentials are required.
This position assists individuals with serious emotional disturbance and/or mental health disorder(s) in gaining access to necessary medical, behavioral, emotional, social, educational, and other services appropriate to their needs. Case Managers are responsible for providing ongoing monitoring and follow-up of the provision of services included in the individual's Person Centered Plan to ensure effectiveness and make necessary changes as identified by the treatment team. Case Management involves locating, obtaining, coordinating, and monitoring medical, behavioral, emotional, social, and rehabilitative services and other needed services and supports related to maintaining the person's health, safety, and well-being in the community.
Description of Responsibilities & Duties:
Develop, update, implement and review Person Centered Plan (PCP) with the consumer, family, and treatment team members and ensure coordination of this plan with other plans of the consumer's life (e.g., IEP, 504 Plan, Probationary, etc.). This includes the development of the PCP in coordination with a comprehensive clinical assessment for consumers receiving enhanced benefit services.
Facilitate the person-centered planning team and conducts treatment team meetings (using the System of Care Model for children/adolescents). Person-Centered Thinking and planning philosophies must meet the needs of the consumer and their family. Reviews person-centered plan goals and interventions with families, consumers, natural supports, and other agencies in participation of the consumer's life and whom the consumer chooses to have as part of the Person-Centered Team. This includes a review and documentation of a consumer's progress, and regression, tracking outcomes as identified by Agency/service definition, and ensuring identifying needed services are based on the assessment for each consumer being served.
Provide ongoing updating of each section of the PCP after each team meeting, including but not limited to updating goals, interventions, services, progress toward goals, regression, as well as updating the Crisis Plan.
Responsible for coordinating and scheduling treatment team meetings. This includes a minimum of 1 treatment team meeting per 30 days.
Perform and complete all necessary paperwork for utilization review and reauthorization using the principles under the Continued Stay and Discharge Criteria guide to the review of services.
Make sure all authorizations are current and have been authorized. Track and complete authorizations as needed to ensure there is no lapse in service
Conducts referral/linkage activities to connect consumers with medical, behavioral, emotional, social, and other programs, supports, and services to address identified needs and achieve goals specified in the Person Centered Plan.
Assist family/consumer in locating &/or arranging transportation services and natural resources and develop skills to achieve greater independence or function with natural supports. Locate and coordinates sources of assistance and services to ensure the individual receives available natural and community support and resources.
Job Type: Full-time
Pay: $39,000.00 - $43,000.00 per year
