Job Summary:
Join our compassionate team as a Primary Support Professional, where you will play a vital role in enhancing the lives of individuals receiving residential services. You will collaborate closely with the Program Coordinator to implement Individual Support Plans (ISPs) effectively. This position offers the opportunity to make a meaningful difference in the lives of clients while developing essential skills in support coordination, communication, and team collaboration.
In this role, you will work directly in the homes of clients, providing support services and ensuring their daily activities align with their individual needs. Key Responsibilities:
- Work directly in the home or community with assigned clients, as designated by the Program Coordinator and/or the Regional Director.
- Participate in Individual Support Plan meetings and RN visits or service monitoring visits when possible.
- Train staff on each client’s medical needs, personal preferences, and compliance.
- Oversee daily residential notes, goals, and health tracking to ensure quality care.
- Review the physical condition of the home to maintain health, safety, and cleanliness standards.
- Ensure safety kits are present and complete, including First Aid and Emergency Preparedness Kits.
- Schedule staff or cover shifts for personnel who are sick or absent.
- Collaborate with the Program Coordinator on follow-up to monthly RN visits and special diet trainings.
- Ensure daily exceptions are documented and submitted to the Program Coordinator at the end of each month.
- Other duties as assigned.
Education, Experience, and/or Credential Qualifications:
- Must be 18 years of age; must have a high school diploma or GED.
- One year of experience in the field of intellectual/developmental disabilities, or successful completion of training in the Missouri Quality Outcomes approved by the Division of DD regional office may substitute for experience.
Additional Qualifications:
- Must have a reliable means of communication.
- Must possess skills to read and understand written materials.
- Must possess skills to write clearly and legibly.
Physical Requirements:
- ADA Consideration - Sedentary work: Exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently to lift, carry, push, or pull, or otherwise move objects, including the human body.
- Sedentary work involves sitting most of the time but may involve walking or standing for brief periods.
