Supportive Living Services is currently seeking responsible, caring, and dedicated people for Part-time Direct Support Professional (Caregivers) positions. Caregivers in our community based program are offered a diverse and flexible schedule. A valid driver's license and the ability to transport individuals via a personal vehicle are requirements for this position. SLS offers a comprehensive orientation and training program to all new employees, no past experience working with individuals with intellectual disabilities or autism spectrum disorder is required.
Starting rate of $16.50 per hour
Paid training
Flexible work schedule (Days and evenings available)
401 K available for part time and full time staff
Requirements:
- Must be 18 or older
- High School diploma or equivalent
- Valid driver's license and insurance
- Able to provide transportation utilizing personal vehicle (Generous mileage reimbursement included)
- Proof of vehicle inspection
- Registration card and vehicle insurance card
Essential Duties and Responsibilities:
Relate, interact, and communicate positively with the individuals.
Support, and assist the individuals in enhancing skills that will further their independence, increase their community involvement, and improve their quality of life.
Participate with the Program Director in the completion of individual assessments.
Maintain records of individual’s progress in specific program areas.
Assist and or instruct individuals in activities of daily living (meal prep, house cleaning, grocery shopping, laundry and general maintenance) self-care skills (bathing, dressing, personal hygiene); safety and prevocational skills when appropriate.
Document each encounter with the individual as it relates to their lSP outcomes.
Motivate and assist the individuals in all areas designated in the ISP as outcomes.
Display a positive, flexible, and cooperative attitude toward supervisors, coworkers and the general public.
Assist individuals with medical appointments.
Report or communicate all pertinent medical information on a continuity of care form to all appropriate persons (i.e. Program Supervisor(s), Medical Department, Supports Coordinator and Family Members).
Assist individuals in development of skills to independently advocate for themselves. Procure necessary services as needed.
Meet with Program Supervisor(s) as needed to clarify job responsibilities, discuss individual needs.
Attend trainings and in-services as required.
This position requires you to frequently stand, walk, climb stairs, squat, and bend. Must routinely lift 20 lbs or more. Must also assist with lifting individuals, and have the ability to monitor vital signs.
