What are the responsibilities and job description for the Activities Director position at Susquehanna Nursing and Rehabilitation Center?
The Activity Director is responsible for carrying out the facilities’ activity programs for residents. Develops, implements and evaluates resident activity programs and individualizes comprehensive care plans, supervises and evaluates activities personnel, and volunteers.
ESSENTIAL DUTIES & RESPONSIBILITIES:
Develops monthly activity schedule for residents offering programs that meet the interests and capabilities of individual residents. Plans age and developmentally appropriate group activities for residents.
Follows resident care plans and documents all activities residents participate in including family visits ensuring compliance with federal and state regulations.
Reviews and updates resident care plans as necessary minimally quarterly
Provides statistical information as required by the NYS DOH.
Acts as an active member of the interdisciplinary team and attends all resident care conferences (or designee). Acts as the liaison for resident council.
Directs and supervises the activities staff and agency volunteers. Orients new activity staff
Participates in public relations and community relations events
Others duties as requested by Supervisor
Pay Rate: $19-$23 per hour, depending upon experience
