JOB ACCOUNTABILITIES:
CLERICAL.
• Interviews patients on a daily basis for information required to create the Birth Certificate.
• Obtains copies of mother’s medical records on a daily basis and extracts pertinent information from the records.
• Obtains copies of baby’s medical records on a daily basis and extracts pertinent information from the records.
• Enters data into the database to complete Birth Certificate Worksheets, Fetal Death Worksheets and then generates Birth Certificates and Death Certificates.
• Interfaces with Social Services, Department of Public Health and mortuaries to complete and process Fetal and Neo-natal Death Certificates and disposal permits.
• Enters data into the database to complete Discharge Summary and Diagnosis
• Interfaces with physicians and their office staff through sending Discharge Summaries to them and obtaining signatures on required forms/certificates. Interfaces with Medical Records by providing them with disks containing patient Discharge Summaries, exported statistics, and ICD-9 codes.
• Interviews appropriate patients and files forms with the California Department of Social Services regarding the Paternity Opportunity Program. Provides parent education regarding the same. Provides or obtains interpreter services as necessary to facilitate communication with parents.
• Batches, edits, maintains log for and mails certificates to Department of Public Health for registration weekly. Generates daily and monthly reports and logs.
• Provides parent education regarding Birth Certificate, Social Security number automatic request application and Birth Verification letter use.
• Assists parents in need of rushing Passport applications, acting as a liaison between physicians, parents and the State of California.
• Prints Social Security information sheets and Birth Verification letters for parents upon request.
• Provides forms and directions in completing and filing Supplemental Name Forms for parents not naming their baby or babies in the hospital.
• Assists parents in locating information regarding adoption, fertility issues, implements State of California policies as they pertain to the department.
EDUCATION:
- Equivalent experience will be accepted in lieu of the required degree or diploma.
- HS Diploma: High School Diploma or General Education Diploma (GED) or equivalent education/experience
TYPICAL EXPERIENCE:
- 1 year recent relevant experience.
SKILLS AND KNOWLEDGE:
- Knowledge of english language w/ written and verbal communication skills.
- Organizational skills with ability to prioritize, problem solve and meet deadlines.
- Interpersonal and customer service skills.
- Basic math skills demonstrating accuracy and attention to detail.
- Basic keyboarding and computer skills.
- Ability to gather data to complete reports on a timely basis.
PAY RANGE: $ 27.44 - $ 34.30 / HOUR
The salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled. Compensation takes into account several factors including but not limited to a candidate’s experience, education, skills, licensure and certifications, department equity, training and organizational needs.
