Summary:
Provides client support through milieu management including individual and group interventions, daily activity scheduling, crisis care, and care coordination.
ESSENTIAL FUNCTIONS:
- Responsible for the daily groups on the unit and supporting unit staff in running effective groups through mentoring and providing curriculum.
- In collaboration with the multidisciplinary team, ensures that the clients have an activity and daily schedule that supports the entire milieu for the afternoon/evening and prepares the schedule for the entire week and weekend.
- Communicates directly with clients who are experiencing crisis to assess mental status, current level of functioning, and the person’s wishes and goals for crisis stabilization.
- Assists the team in developing a crisis stabilization plan with the client and family or guardian, based on a comprehensive assessment that includes mental status and risk assessments along with multidisciplinary team.
- Communicates with the family as needed on a weekly basis.
- Acts as the liaison with local police department, emergency departments, crisis teams, and inpatient units, school, supervisors and leadership.
- Collaborates with individual’s providers and natural supports.
- Transports clients as needed.
- Implement and demonstrate a SAMHSA trauma-informed and strengths-based approach in interventions.
- Follow proper behavioral interventions including Safety Care procedures, including preventative behavior supports and physical interventions.
- Meets recordkeeping requirements by the timely completion of safety plans, crisis stabilization assessments and by maintaining current progress notes, logs and other information as required in the client’s chart.
- Assists in the intake and discharge planning of clients as assigned by the supervisor.
- Communicates and liaises with the crisis triage department for new referrals
- Attends all treatment team and service planning meetings.
- Minimum Associate’s Degree. Bachelor’s Degree preferred
- BHP required within 6 months; MHRT-C or MHRT-CSP Eligible
- Maintain a valid state driver’s license.
- M ust be able to speak, read and write English fluently.
- Minimum of two years relevant experience required. Mobile crisis experience or inpatient experience preferred.
