Job Summary:
Partners In Home Care is seeking a highly motivated and dynamic Hospice and Palliative Care Director to lead our comprehensive care program. This role is responsible for overseeing daily operations, strategic planning, financial management, patient safety, service quality, business development, and community relations. The Director supervises and supports an interdisciplinary team, including nurse practitioners, nurses, social workers, and office staff. This position reports directly to the CEO and ensures that all aspects of hospice and palliative care services meet the highest professional and regulatory standards.
Essential Duties and Responsibilities:Financial Management:
- Develop and oversee budgets for all Hospice and Palliative Care operations.
- Monitor financial performance and implement cost-effective strategies to maintain quality care delivery.
- Ensure compliance with financial regulations and reimbursement policies.
Compliance and Quality Assurance:
- Maintain knowledge of CMS regulations and Conditions of Participation for Hospice and Palliative programs.
- Ensure ongoing survey readiness and compliance with federal, state, and accreditation standards.
- Develop and implement policies and procedures to uphold legal, ethical, and best practice standards in service delivery.
Program Development & Operations:
- Establish evidence-based patient care standards for Hospice and Palliative services.
- Lead initiatives for program growth, including the establishment of new care models and services.
- Oversee the development and implementation of business plans, operational budgets, and new policies.
- Collaborate with community organizations to enhance service accessibility and reach.
Talent Management & Leadership:
- Assess staffing needs and resource allocation to meet clinical objectives.
- Develop performance standards and growth pathways for staff to ensure high-quality care delivery.
- Foster an environment of excellence, positive change, and team engagement.
- Ensure a safe work environment and adherence to workplace safety standards.
Strategic Growth & Community Relations:
- Develop clear and transparent communication channels within and across programs.
- Partner with internal teams to create growth strategies and enhance market share.
- Represent the organization in community partnerships and advocacy efforts.
- Participate in collective bargaining activities, ensuring a fair and effective workforce strategy.
Qualifications & Requirements:Education:
- Required: Bachelor's degree in Nursing (BSN), Healthcare Administration, Public Health, or a related field.
- Preferred: Master’s degree in Nursing (MSN), Healthcare Administration, or Public Health.
Experience:
- Minimum of 3 years of leadership experience in Hospice and Palliative Care or a similar healthcare setting.
- Strong background in operations management, financial oversight, and compliance.
- Experience in business development and community outreach preferred.
Licensure & Certifications:
- Required: Active Registered Nurse (RN) license in Montana or compact state licensure.
- Required: Certification in Hospice and Palliative Nursing (CHPN) or ability to obtain within the first year.
- Preferred: Certification in Hospice and Palliative Care Administration (CHPCA).
- BLS (Basic Life Support) Certification required.
- Valid Driver’s License (for potential community outreach and home care oversight).
