Job Description:
Care Management within the Department of Psychiatry includes a collaborative practice with patients, caregivers, nurses, social workers, physicians, payers, support staff, other practitioners, and the community. Social workers facilitate communication and care coordination along a continuum through effective transitional care management. Recognizing the patient's right to self-determination, the significance of the social determinants of health, and the complexities of care (barriers to coping, abuse, mental health, substance use, lack of resources, psychosocial issues, etc.).
Required Qualifications:
- Master's Degree in Social Work.
- Coursework/Training: All LCSWs must follow relevant guidelines for required designations and maintain current status.
- Clinical Social Worker License upon hire.
- 2 years of experience in a healthcare-related setting with a psychiatric mental health patient population, preferably in an inpatient setting.
- Computer experience.
- Designated as a Behavioral Health Clinician to perform evaluations within 18 months of hire.
- CPI certification (nonviolent crisis intervention training) within 1 year of hire.
Preferred Qualifications:
- Discharge planning experience.
