What are the responsibilities and job description for the Patient Navigator Part-time- Central Location position at Swope Health Services?
The Part-Time Patient Navigator will utilize a variety of methods to reach out to patients with the purpose of directly improving health outcomes by connecting patients with healthcare and resources through numerous funding sources. The Marketplace Navigator works collaboratively with other team members to ensure patients receive exceptional service and care.
Primary Functions :
Conduct public education activities to raise awareness about the Exchange.
Attend outreach events.
Provide information in a manner that is culturally and linguistically appropriate to the
needs of the population served by the Exchange, including individuals with limited
English proficiency, and ensuring accessibility and usability of Navigator tools and
functions for individuals with disabilities.
Provide referrals to any applicable office of health insurance consumer assistance or
health insurance ombudsman for any enrollee with a grievance, complaint, or question
regarding their health plan, coverage, or a determination under such plan or coverage.
Comply with applicable training and conflict of interest standards.
Obtain authorization of applicants for coverage available through an Exchange
application prior to accessing their personally identifiable information.
Provide targeted assistance to serve underserved or vulnerable populations, as
identified by the Exchange, within the Exchange service area.
Educational Requirements :
High school graduate or G.E.D. required
Experience & Qualifications :
Bilingual English / Spanish
Excellent organization skills
Excellent communication skills and phone etiquette
1 years office experience or patient experience
Experience with an electronic medical record; intermediate computer proficiency (multi-task through multiple applications including Microsoft.
