As the Home Care Nurse for PACEKC, you will be part of a team of caregivers that deliver care to the people who need it the most. The Home Care Nurse manages the clinical care needs of participants in the home environment. The home care nurse will educate participants, caregivers, and family regarding care needs to maximize the independence and quality of life for our participants. In your role you will:
- Provides skilled nursing services in accordance with the plan of care.
- Maintains continuity of participant care by communicating effectively with team members and collaboration with interdisciplinary team.
- Evaluates and routinely re-evaluates the care needs of the participant and makes recommendations to interdisciplinary team for needed changes in the plan of care.
- Educates the participant, caregiver, family, and associates involved in the participant care to promote independence and quality of life of the participant.
- Communicates with the primary provider for direction and needed orders.
- Assesses the home environment for safety needs and or issues that may arise. Communicates with interdisciplinary team for collaboration of plan of care.
- Assesses skin conditions and wounds and report changes or problems to the primary provider immediately.
- Assesses participant behaviors and cognitive abilities and recommends appropriate steps or interventions to interdisciplinary team to promote intendance, safety, and quality of life.
- Participates in after-hours and weekend on-call rotation ensuring availability of 24-hour care delivery to participants.
- Assists participant or caregiver in setting up medication regime in the home.
- Documents grievances and follows the grievance policy for all participant complaints.
- Stocks and prepares supplies for home care needs of the participants as needed.
- Maintains up-to-date and appropriate documentation in the participant's electronic medical record.
- Participates in the interdisciplinary team meetings, communicating participant changes, collaborating on care planning decisions and coordination of 24-hour care delivery.
- Participates in in-service training and staff meetings as required.
Successful Candidates Will have:
- Licensed to practice as a RN or LPN in the State of Missouri.
- Medical clearance for communicable diseases and up-to-date immunizations, before having direct participant contact.
- Current driver's license, auto insurance, and dependable transportation.
- (2) years' experience as a Home Care I Home Health Nurse.
- (1) year working with the elderly population
- Current American Heart Association Basic Life Support (BLS) with Automated External Defibrillator (AED) certificate or the ability to obtain within two weeks of hire, before having direct participant contact
