Symbii Home Health & Hospice Preston is seeking an Office Assistant to join our growing Home Health & Hospice team! We are passionate about providing quality home health and hospice to folks who need it, and we are looking for an Office Assistant who shares our drive, vision, and commitment.

As a vital member of our team, our office assistant is responsible for ensuring that our agency is organized and running efficiently.

Duties and Responsibilities:
  • Ensures compliance with all state, federal, and Joint Commission referral/intake regulatory requirements.
  • Establishes and maintains positive working relationships with current and potential referral sources.
  • Ensures seamless transition of patients to home care by providing direct oversight of patient education and preparation for home care, plan of care initiation, and coordination of care with multiple service providers.
  • Builds and monitors community and customer perceptions of Symbii Home Health and Hospice as a high-quality provider of services.
  • Gathers, collates, and reports referral statistics including key customer referral trends.
  • Maintains comprehensive working knowledge of Hospice Home Health and Hospice contractual relationships and ensures that patients are admitted according to contract provisions.
  • Maintains comprehensive working knowledge of community resources and assists referral sources in accessing community resources should services not be provided by Symbii Home Health & Hospice.
  • Utilize computerized records program to process, store and retrieve health information.
  • Analyzes and reviews medical records for completeness and accuracy of documentation according to specified standards.
  • Assures agency is in compliance with agency policies and procedures related to medical records and meets documentation requirements of federal and state regulatory agencies.
  • Assures agency complies with the Health Information Portability and Accountability Act.
  • Maintains records related to HIPAA compliance.
  • Maintains clinical records by creating files, organizing and maintaining records to meet standards of practice.
  • Confers with doctors, nurses and other health personnel to assure complete, current and accurate medical records.
  • Files documentation accurately in patient records.
  • Keeps filing current.
  • Audits records and prepares identifies inaccuracies or delinquencies keeping management updated on trends.
  • Follows-up with appropriate individuals to resolve incomplete records.
  • Retains old records in accordance with medical record retention requirements and arranges for file destruction when appropriate.
  • Work as part of the branch office team, assist with handling phone calls, attend team meetings as needed, support other functions when reasonable and needed.
  • Responsible for scanning and attaching medical records into the appropriate client folders in the company's electronic medical records.
  • Schedule and appropriately document patient schedules in a timely manner.
  • Ability to multi-task in a fast-paced environment.
  • Participate in coordinating care with management and patient interdisciplinary team.
  • Provide effective communication to patients, staff members, other health care professionals and referral sources.
  • Respond to agency and patient needs in a professional and creative manner.
Job Requirements:
  • Must have healthcare experience, preferably in referrals/intake in a home health or hospice environment.
  • A nursing background is preferred but not required.
  • Demonstrates good communication, negotiation, and public relations skills.
  • Demonstrates autonomy, assertiveness, flexibility and cooperation in performing job responsibilities.
  • Strong written and oral communication skills in person and over the phone.
  • Ability to read and interpret documents such as safety rules, procedure manuals, and regulatory guidelines related to home health & hospice agencies.
  • Ability to write routine reports and routine business correspondence. Ability to communicate effectively with patients, families and co-workers.

  • Technical aptitude to know or learn and master the agency’s clinical software and work with Microsoft Office Suite software.
