Role Description
This is a full-time on site role for a Lead Care Coordinator at Synergy Homecare North Atlanta. The Lead Care Coordinator will be responsible for coordinating care services, managing cases, communicating with clients and caregivers, providing home care support, and delivering exceptional customer service.
Qualifications
- Care Coordination and Case Management skills
- Strong Communication and Customer Service abilities
- Experience in Home Care services is ideal but not required.
- Ability to multitask and prioritize tasks effectively
- Empathetic and compassionate approach to caregiving
- Knowledge of Alzheimer's and dementia care practices would be a bonus.
- Proficiency in Microsoft Office and care management software
- Previous experience in a similar role is a plus
