Job Summary:
Develops, implements and evaluates resident activity programs and individualizes comprehensive care plans, supervises and evaluates activities personnel, companion aides and volunteers
ESSENTIAL DUTIES & RESPONSIBILITIES:
- Develops a monthly person-centered activity schedule based upon individual and group needs, interests, capabilities and cultural backgrounds
- Responsible for the selection and evaluation of activities and program staff and volunteers; as well as recognition of staff and volunteers
- Maintain an activity attendance record for each resident
- Initially develops with the resident and prepares for each resident a written plan for individual, group and independent activities in accordance with their needs, interests and capabilities
- Identify and document in the resident’s care plan, the activity interests, needs, and concerns of the resident. Care plans are reviewed and updated as necessary but at least quarterly
- Documents all interaction with resident and/or family in the assessment, care plan and progress notes as required by federal and state requirements
- Include in the resident’s medical chart a periodic assessment of the resident’s participation in and response to the activities program
- Resident Council liaison
- Participate as an active member of the interdisciplinary team and attend all resident care conferences (or designee)
- Assigns duties and supervises all activities staff and assigned volunteers
- Functions as facility volunteer coordinator (interviewing, scheduling, recruitment, retention and recognition).
- Develops schedule for activities staff and approves electronic time cards for staff
- Participates in public relations/marketing /community relations events
- Assess needs and maintain an adequate supply of materials to implement activity programs
- Maintains resident rights
- Maintains HIPAA privacy, security and confidentiality standards
- Follows all established policies and procedures
- Follows universal Precautions and Infection Control techniques
- Maintains Corporate Compliance standards
- Contributes to facility Continuous Quality Improvement initiatives
- Follows safety procedures by keeping areas free of hazardous conditions
- Economical and careful with supplies and equipment
- Attends all required in-services
- Comes to work as scheduled and consistently demonstrates dependability and punctuality
- Comes to work in neat, clean attire and consistently presents an appropriate professional appearance
- Actively participates in Quality of Life Subcommittee and other Committees as directed by the Administrator
- Performs any other necessary duties as assigned by Supervisor
- This individual continuously exhibits and fosters a spirit of teamwork, positive attitude and conflict resolution among co-workers
- Coordinates activities in conjunction with other departments, therapy, nursing, dietary and social work to make residents feel safe and empowered

- Identifies actual or potential problems and devises an action plan for resolution. Implements solutions and monitors outcomes using judgment skills to choose the best course of action

- Acts as a source of direction to visitors, staff and external populations, and volunteers

- Promptly reports all needs and concerns to the Supervisor or designated alternate
- Work with Human Resources to integrate new employees into facilities
- Communicate with resident’s family regarding how resident is doing in activities
