Position Overview
Support Consultants provide education and guidance to individuals participating in the Community First Services and Supports program. They assist in writing service delivery and quality review plans, evaluate service utilization, and offer recommendations to improve the quality of services. Meetings with participants may take place over the phone or in their homes.

Duties & Responsibilities
- Conduct home visits to educate program participants on program models, rules, processes, and policies.
- Assist with writing service delivery plans and quality review plans.
- Evaluate service utilization according to approved plans and provide recommendations for improvement.
- Organize and manage a caseload effectively, ensuring compliance with contractual timelines.
- Maintain timely documentation, ensuring visit notes are completed and uploaded on the same day.
- Represent the organization positively to participants and stakeholders.
- Ability to visit a diverse population of participants in their homes, which may include various home environments.
- Spend at least 50% of working hours conducting home visits, with flexibility to increase as needed.
- Organize caseload efficiently to maximize productive use of travel time.
- Travel up to 50 miles or more to participants’ homes.
- Maintain a satisfactory driving record and have a reliable, personally insured vehicle.
- Proficiency in using computer systems and Microsoft Office software.
- Strong time management and organizational skills to manage caseload independently.
- Ability to prioritize tasks in a dynamic, fast-paced environment.
- Excellent verbal and written communication skills.
- Understanding of self-direction principles and ability to explain them to others.
- Team-oriented mindset with the ability to collaborate effectively with colleagues.
- 2-3 years’ experience working with vulnerable populations in their homes.
- BA or BS degree in social work, case worker, sociology, psychology, or related field preferred.
- 2 years’ experience in social services and health care system; related state programs/supports and federal programs (e.g. Medicare, Medicaid)
- Familiarity with electronic records preferred.
