Key Responsibilities:
- Engage individuals with mental illness to support their transition to independent living.
- Conduct group meetings on Wellness Management.
- Develop and review service plans for all residents.
- Assess, correct, and report on living conditions within apartments and personal living spaces.
- Provide case management services and assist residents in setting and achieving rehabilitation and housing support goals.
- Collaborate with residents to develop Housing Support Plans as needed.
Qualifications:
- Education: Bachelor’s Degree in human services or a related field required.
- Experience: Minimum of two years of experience working with individuals with mental illness.
- Skills: Strong computer literacy in word processing and excellent writing capabilities for developing treatment plans.
