Job Overview – Medical Case Manager (Homeless Outreach):
Support our client as a Medical Case Manager (Homeless Outreach) in Stroudsburg, PA. In this impactful role, you’ll work within a homeless day center to provide values-driven support and advocacy for program participants. Collaborate with a multidisciplinary team to connect individuals with resources, help them achieve their goals, and promote their overall well-being.
Compensation: $19/hour
Location: Stroudsburg, PA
Schedule: Monday to Friday, 8:00 AM to 4:00 PM
Responsibilities as the Medical Case Manager (Homeless Outreach):
- Participant Advocacy: Develop individualized goal plans in collaboration with participants, providing support, advocacy, and connections to resources for housing, employment, and other needs.
- Community Engagement: Build and maintain partnerships with local agencies to connect participants with vital services and resources.
- Service Delivery: Conduct intake assessments, provide site-based services, and focus on securing shelter, employment, and access to necessary documents.
- Documentation & Reporting: Complete all required documentation and assessments, ensuring compliance with funder and licensing standards.
- Team Collaboration: Work within a multidisciplinary team to deliver holistic, values-driven care and support.
Qualifications for the Medical Case Manager (Homeless Outreach):
- Education: Bachelor’s degree in Human Services or a related field required.
- Experience: At least 2 years of experience in Homeless Services, Behavioral Health, or Addiction Recovery (1 year of case management experience preferred).
- Skills: Demonstrates strong organizational and interpersonal skills, with a commitment to participant advocacy and holistic care.
- Technical Skills: Possesses intermediate computer proficiency, ensuring efficient task management and data handling.
- Licensure: Holds a valid driver’s license, meeting transportation requirements for the role.
- Certification: Maintains CPR certification or has the ability to obtain it promptly.
