Job Overview - Medical Case Manager:
Our trusted client in Brooklyn, NY, is seeking a Medical Case Manager to support individuals with mental illness in maintaining housing and achieving independence. This temp-to-perm role includes teaching skills, connecting clients to services, and leading wellness group meetings.
Compensation: $24/hour
Location: Brooklyn, NY
Schedule: Monday to Friday, 9 AM to 5 PM
Responsibilities as the Medical Case Manager:
- Client Support: Engage clients with mental illness, assist in community living, and develop housing support plans.
- Group Facilitation: Conduct group meetings focused on wellness management and recovery.
- Service Planning: Complete service plan reviews and regularly assess living conditions for residents.
- Case Management: Collaborate with residents to set and achieve rehabilitation and housing support goals.
Qualifications for the Medical Case Manager:
- Education: Bachelor’s Degree in human services or a related field required.
- Experience: 2 years working with individuals with mental illness.
- Skills: Computer literacy, and strong writing capabilities for treatment plans.
- Language Skills: Bilingual (English/Spanish) preferred.
