What are the responsibilities and job description for the Transitional Living Program Case Manager position at Tahoe Youth & Family Services?
JOB SUMMARY: The Case Manager will work within TYFS’ Transitional Living Program (TLP). The TLP Case Manager reports to the Program Manager and assists the Program Manager in implementing the TLP Program. The TLP provides transitional housing at centrally located scattered site apartments to homeless youth ages 16 to under 24 years of age. Residents receive safe, stable housing, staff support and supervision, connections to needed services and life skills training. The TLP program provides intensive, basic, and independent living skills assessment, training, supervision, counseling, referrals, and support, as needed. This position is a full-time, exempt position which includes medical, dental and vision benefits and accrues vacation and sick leave in accordance with the agency's Personnel Policy and Procedures in the Employee Handbook.
RESPONSIBILITIES INCLUDE:
- Maintain a caseload of TLP residents from Intake and assessment to Aftercare, program activities, procedures and policy are centered around youth voice and choice. This reflects our philosophy that only when a person is empowered can they make real change.
- Use a Positive Youth Development approach, combined with a harm reduction model and a trauma-informed care protocol.
- Assist participants in identifying and assessing immediate and long-term needs, including safety, well-being, and self-sufficiency.
- Determine eligibility and gather intake info, and assess strengths, needs, and interests.
- Develop the TLP Graduation & Goal Plan with the youth
- Provide referrals utilizing community resources.
- Provide effective case management.
- Advocate on participant's behalf with other service providers, landlords, the legal system, etc.
- Assist participants in entering TLP apartment, orienting and supporting them to set up a safe, healthy home.
- Provide skill-building in a full range of independent living skills ( in both one-to-one and small group training).
- Provide assistance in the areas of health promotion (physical, cognitive, emotional/mental), education, and employment.
- Place and monitor participants in workplace apprenticeships based upon the participant's interests and skills.
- Assist youth in meeting their goals and transitioning to their own housing upon graduation from the TLP.
- Ensure that all agencies, Federal grantor, and CA State mandates, policies, & procedures are followed throughout the course of work.
- Maintain required documentation on the program and all participants.
- Maintain high ethical standards when carrying out all duties and maintain strict adherence to all confidentiality, legal, and agency guidelines in all areas of service provision.
EDUCATION & PROFESSIONAL EXPERIENCE REQUIRED FOR THE POSITION:
At least two years’ experience working with youth or young adults in a community setting or possesses additional related work experience.
- Demonstrated ability to work effectively with difficult-to-reach service populations, especially homeless/disenfranchised youth and young adults, in a multi-disciplinary human service setting. Experience planning and conducting individual and group activities.
- Excellent organizational skills and the ability to deal with multiple tasks simultaneously.
- Qualities of a counselor & social worker, though certification for these roles is not mandatory.
- Experience working with social agencies, schools, and non-traditional educational programs.
- Must possess basic computer literacy skills. Familiarity with using the internet.
- Must have a reliable car, and a valid, clean driver's record.
- Must be able to work some evenings.
- Must be available to be on emergency on-call basis

RESPONSIBILITIES
1. Develop an Individualized Service or Treatment Plan for each client that include evidence-informed strategies to assist with the trajectory of achieving sustainable living for every youth receiving services.
2. Record Keeping and Case management for Infant/Toddler: Case Manager will create a separate file for the young person and each infant or child. This should include a record of well-care visits, sensory and developmental screenings and assessments.
3. Develop a Service Coordination Plan that will coordinate with the local Department of Housing and Urban Development (HUD) funded Continuums of Care (CoC) to ensure youth have access to all available resources including adult mainstream services and youth services.
4. Must also have a suitable referral plan based on an assessment of youths.

5. Program must have a referral plan based on an assessment of your system of services which must include social services, law enforcement, educational services, vocational training, welfare, legal services, anti-trafficking agencies health care programs, including health insurance options; affordable childcare; and/or child education programs.
6. Provide every youth a written transitional living plan that meets their level of need for a safe and stable living environment after program exit.
7. Provide an Aftercare Plan for each youth. This plan must ensure each youth has a safe and appropriate exit plan when leaving the program. The aftercare plan should track the youth’s housing status, education services, and the rate of participation and completion of the services in the plan beyond 3 months, if services are still provided.
8. Provide written monthly progress reports to the Supportive Services Manager.
9. Participate in regularly scheduled staff meetings and related functions.
