Key Responsibilities
- Provide comprehensive primary and consultative care for geriatric patients.
- Develop personalized treatment plans addressing physical, cognitive, and psychosocial needs.
- Collaborate with a multidisciplinary team, including nurses, social workers, and therapists, to optimize patient outcomes.
- Perform geriatric assessments and manage age-related conditions such as dementia, osteoporosis, frailty, and polypharmacy.
- Educate patients and families on preventative care, chronic disease management, and end-of-life planning.
- Participate in quality improvement initiatives and contribute to the development of geriatric care programs.
- Maintain accurate medical records and ensure compliance with healthcare regulations.
- M.D. or D.O. degree from an accredited institution.
- Board certification or eligibility in Geriatric Medicine and Internal Medicine or Family Medicine.
- Valid medical license (or eligibility) in New York State.
- Experience in long-term care, palliative care, or hospital-based geriatrics is a plus.
