Responsible for providing prevention and education services in a variety of settings including on the phone. May implement prevention services over a varied group of topics including substance abuse, mental health, bullying, suicide and other areas as necessary.
Position Description:
Identifies and assesses needs of callers/clients
Provide crisis intervention, referrals and linkage to community resources
Utilizes supportive techniques and/or educational methods that are appropriate to caller and topic
Completes service documentation accurately and timely in electronic health record
Provides both prevention and education services
Facilitates referral to and/or provides appropriate level of care
Develop, define and implement realistic and effective goals and plans for interventions
Collect, track and evaluate program data for fidelity and service quality
Adhere to professional standards as outlined by protocols, rules and regulations
Other duties as assigned

Required Knowledge, Skills and Abilities:
Skill in prevention and education techniques and interventions
Recognize unusual or threatening conditions and take appropriate action
Effective verbal and written communication skills
Ability to interact in a calm and compassionate manner
Ability to collaborate/interact with wide variety of people
Knowledge of code of ethics, conflict of interest and regulatory requirements
Demonstrate professional speech and behavior
Detach from client issues and view objectively
Job Requirements:
High School diploma or GED
Prefer experience in social services environment or related education
Essential Functions/Physical Demands:
Positional: Driving in accordance with job duties assigned. Occasional standing and walking. Constant sitting.
Gross Mobility: Rare climbing, or crawling. Infrequent balancing, stooping, kneeling, or crouching. Frequent to constant reaching, handling and fingering.
Sensory: Rare tasting/smelling. Infrequent use of color vision. Occasional use of far visual acuity, depth perception, and field of vision. Frequent use of near and midrange visual acuity, and visual accommodation. Frequent to constant talking.
