Position Overview:
We are seeking a dedicated and compassionate Case Manager to work under the supervision of the Regional Director of Clinical Services. This role will involve implementing client services in alignment with Talbot Health Services’ mission, vision, and values. The ideal candidate will be committed to providing exceptional care and support, ensuring our clients receive the best possible services. This is an hourly position, with a schedule designed to meet the required duties.
Key Responsibilities:
- Monitor clients’ mental health, substance use, and physical well-being; communicate any changes to the treatment team for appropriate intervention.
- Work directly with clients and their families to identify needs and connect them with necessary resources.
- Coordinate with community resources (e.g., schools, probation officers, outpatient mental health providers) to ensure seamless care.
- Schedule and facilitate client appointments to address mental health and substance use needs.
- Achieve assigned productivity goals.
- Document services provided within 48 hours.
- Ensure adherence to professional ethics, agency policies, and state/federal laws.
- Maintain client records and charts with up-to-date information.
- Participate in weekly supervision and bi-weekly treatment team coordination.
- Conduct basic living skills assessments for all clients.
Requirements:
- Active Ohio CDCA license
- High school diploma, GED, or equivalent
- Minimum of three years of relevant experience
- Valid Ohio driver’s license with a good driving record
Preferred:
- Bachelor’s degree in a related field
- CPR/First Aid/AED certification
- Crisis Prevention/De-escalation training
Physical Requirements:
- Must be able to lift up to 15 pounds.
- Ability to work in facilities or community-based settings without elevator or mechanical support.
- Prolonged periods of sitting at a desk and working on a computer.
