Requirements
- Must have knowledge and experience with Epic EHR.
- Must be open to rotating weekends.
- Assist with UR transmission, handling a high volume of faxes and voicemails, and data entry. Epic knowledge is preferred.
- Coordinate and implement the Transition of Care (TOC) / Discharge (DC) plan for ambulatory patients, prioritizing and coordinating the plan across the continuum of care.
- Collaborate with Case Managers and Social Workers to develop and execute TOC plans, ensuring timely discharge.
- Monitor progress towards TOC goals, escalating barriers to Case Managers and Social Workers as needed.
- Coordinate necessary post-discharge referrals and authorizations in collaboration with the Care Coordination (CC) team.
- Facilitate the transfer of patients to appropriate post-acute facilities, including document preparation and transportation assistance.
- Participate in rounds with Case Managers and Social Workers to provide updates and receive direction.
- Deliver the Medicare "Important Message” (IM) and inform patients or facilities of their right to appeal discharge decisions.
- Proactively identify and resolve barriers to timely TOC plans, escalating unresolved issues as needed.
- Actively participate in team huddles and department meetings, contributing to decision-making and planning.
- Maintain accurate, current, and legible documentation according to department standards.
- Enter CC notes in the electronic medical record to capture the status of referrals and communications.
- Provide clerical support, including copying, faxing, scanning, and data entry.
- Demonstrate tact and respect for all customers, building positive relationships with partners.
- Use effective communication skills to resolve issues promptly and productively.
- Comply with confidentiality policies, HIPAA regulations, and department standards when transmitting patient information.
