Working directly with physicians, we are able to manage a variety of diseases and conditions in a homecare setting. Our goal is to help each patient be an active participant in their care. Through education and partnership with each patient and their caregivers, we teach our patients how to effectively manage their therapy and provide ongoing oversight, coordination and support throughout their therapy

Now Hiring Patient Service Representative
Monday - Friday - Rotating Work Schedule (8 : 30 AM - 5 : 00 PM, 9 : 00 AM - 5 : 30 PM, 9 : 00 AM - 6 : 00 PM)
The Patient Service Representative is responsible for delivering exceptional service to customers by being accountable, courteous, understanding, and responsive. They also complete calls to patients for supply, enteral, and medication needs for refills and communicate patient needs to the pharmacy team.
- Adheres to the Corporate Compliance Program, including confidentiality of HIPAA protected healthinformation. Maintains confidentiality of patient information.
- Completes calls to patients to obtain supply, enteral and medication needs for refills.
- Completes Patient Refill Assessment in the system for each delivery.
- Creates a delivery ticket with supplies to be sent to the patient.
- Answers to patient’s questions, resolves complaints and concerns immediately, and facilitates satisfactory resolution.
- Participates in Performance Improvement (PI) Program Coordination; including assisting with the overall development and implementation of the PI plan, assisting in the identification of goals and related patient outcomes, participation and reporting of activities and outcomes, development and implementation of corrective plans.
