Key Responsibilities:
- Conduct comprehensive pediatric assessments and manage acute and chronic conditions
- Provide evidence-based care while supporting wellness and prevention
- Collaborate with internal care teams and coordinate with community providers
- Accurately document in the Electronic Health Record (EHR)
- Participate in quality improvement initiatives and team-based projects
Qualifications:
- Graduate of an accredited nursing and nurse practitioner program (Pediatrics focus)
- Current Connecticut NP license (or eligibility)
- DEA and CT Controlled Substance Registration (or eligible)
- Strong communication, clinical judgment, and documentation skills
- EHR proficiency required
