Job Description:
The University of Virginia Medical Center is seeking an experienced Registered Nurse (RN) Case Manager for a 13-week travel contract in Charlottesville, VA. This position offers a competitive weekly pay of $2300, plus an $800 bonus, and requires the candidate to work 40 hours per week with 5 days of 8-hour shifts.
Key Responsibilities:
Manage and coordinate patient care from admission to discharge in an acute care hospital setting.
Work collaboratively with healthcare teams to optimize patient outcomes.
Ensure appropriate care delivery in a timely and efficient manner.
Document patient progress and interactions through the Epic electronic health record (EHR) system.
Participate in interdisciplinary team rounds, discussing case management plans and interventions.
Perform discharge planning, ensuring continuity of care post-hospitalization.
Work on 2-3 weekends during the 13-week contract period.
Requirements:
Must have:
Acute care hospital inpatient Case Management experience.
Travel or Epic EMR experience.
RN license in good standing.
Ability to work independently in a fast-paced environment.
Must be available to work within a 50-mile radius; local candidates will not be considered.
Schedule:
Full-time, 40 hours/week.
Days: 5 8-hour shifts per week (Monday to Friday).
Weekends: 2-3 weekends during the contract.
