What are the responsibilities and job description for the Military Families Community Health Worker-Tampa Metropolitian YMCA (South & Northwest Tampa Area) position at TAMPA METROPOLITAN AREA YMCA INC?
Under the supervision of the Associate Program Director, Military Families Program and Engagement, the Community Health Worker (CHW) will be responsible for helping Operation Hero and Strong Families participants/communities adopt healthy behaviors and increase access to healthcare. The CHW will work in the assigned YMCA Family Center (North Brandon, Northwest, Spurlino, or South Tampa) to conduct outreach to individuals, health organizations, and local agencies to implement Operation Hero and Strong Families programs. The CHW will assist the Associate Program Director with recruiting and preparing program materials as necessary, interact with the families and Executive Directors at the CHW’s assigned YMCA Center to market the program and collect/report all program outcomes.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:
Program Quality and Growth
- CHW will work in a family center to support the community in improving and maintaining health.
- Meet in one-on-one sessions with enrolled families to: help participants navigate the community resources; share resource maps; and connect families to appropriate food resources, education resources, youth extracurricular activities, and Children’s Board of Hillsborough County (CBHC) resource centers.
- Build relationships with military families to educate program participants about healthcare practices and services and advise them on improving general health.
- Assist families and sites with enrollment in the program and meet requirements of program participation.
- Assist in the community with providing application assistance and referrals as needed and follow up to ensure services are received.
- Maintain confidentiality.
- Facilitates with ability to impartially guide a group with an overall goal of reaching consensus, solving problems or accomplishing tasks, or facilitating a conversation with an individual and/or family in a 1:1 environment.
Operations
- CHW will work in the center year-round to support program participants and alumni 12.5 hours per week, 52 weeks a year.
- Assist in completing other program goals and requirements.
- Input and track data and create progress reports for participants and/or program, including but not limited to, attendance in program, fitness assessments etc.
- Create reports as necessary for leadership and stakeholders.
- Monitor email, properly correspond and follow up with all parties necessary.
- Follow and enforce the Association dress code.
Safety and Risk Management
- Ensure adherence to all YMCA policies and procedures regarding the safety, health and welfare of members, program participants, guests and staff.
- Conduct daily risk management and safety audits in the program space to ensure the safety of all staff and participants.
- Track and store all medical records following HIPAA policies.
