Job Summary
Job Summary: This position supports community maintenance of seriously mentally ill adults who are impaired with challenges of daily living activities. Job incumbent will offer case management services to promote continued growth of independence in the community, and as a liaison with other providers and agencies.
Adhere to the TCC Ch’eghwtsen’ model of service and guiding principle which requires providing timely and effective service along with the ability to interact with others in a way that inspires trust and demonstrates respect, compassion and empathy.

Essential Functions

Representative Duties: Under the direct supervision of the PWH Program Manager, job incumbent will:
1. Provide individual and family case management for clients in coordination with leadership. Assist clients in keeping appointments with mental health and health care providers including providing transportation and monitoring medication.
2. Provide outreach services to patients including routine phone and emergency outreach.
3. Network with area professionals to ensure follow up care and referrals, including transfers of care between psychiatry, therapy, inpatient and outpatient care, primary care clinics and specialty care clinics.
4. Coordinates the scheduling of assessment as needed including the development of the treatment plan using DSM and ASAM criteria.
5. Obtain financial benefits, (SSI and APA), to insure basic needs are met, including arrangement of re-payee or conservatorship if appropriate.
6. Maintain clinical and administrative records in compliance with the State of Alaska Medicaid regulations and any other state and federal regulating statutes as required by program procedure and policy (e.g. Treatment plans, client status reviews, adult daily functional assessments, daily progress notes.)
7. Assist with urgent and routine requests for care, including community and provider requests, gathering pertinent history and records, and assist with triage in collaboration with Directing clinician, Psychiatry, and Medical Providers.
8. Provide prevention, psychoeducation, and support services to patients regarding mental health, substance use, and overall wellness.
9. Coordinate with other private and public mental health, health, and social service providers (e.g. APH, FMH, FCC, Courts) who may provide services to client, to include referrals and assistances with hospital admission, court hearings, and involuntary commitments, transitional and discharge planning.
10. Assist with letters, written evaluations, care summaries, and community resource agencies as needed to provide patient and provider support.
11. Provide assistance with phone calls and follow up calls.
12. Provide on-call coverage for Paul Williams House for emergencies and crisis intervention.
13. Provides transportation of clients using TCC vehicles.

Minimum Qualifications
Minimum Qualifications:
1. Bachelor’s Degree from an accredited university in psychology, counseling, human development or related medical or behavioral health field; Four years of relevant work experience may fulfill this requirement.
2. Must have direct patient care experience, including working with children, adolescents, and families.
3. Experience and education regarding provision of a variety of medical and behavioral health services to adults, children, and adolescents, to include familiarity with service provision, DSM diagnosis, severely emotionally disturbed children, chronic mental illness, dual diagnosis and integrated treatment for substance abuse and mental illness, treatment planning, treatment coordination, case management, consultation, routine charting, Medicaid regulations, and professional reports. Familiarity with Fairbanks area, Interior Regional, and statewide health services in mental health, medical treatment and substance treatment.
4. Three (3) years of experience providing behavioral health services and/or case management preferred.
5. Knowledge of Medicaid third party billing preferred.
6. Must have a driver’s license and a clean driving record.
7. Knowledge and ability to adhere to Health Insurance Portability and Accountability Act (HIPAA) and 42 CFR Part 2 confidentiality regulations.
8. Must demonstrate ethical commitment to mental wellness, recovery and goals of Psychiatry within Tanana Chiefs Conference Health Services.
9. Must pass background check pursuant to Alaska DHSS and federal Indian Child Protection and Family Violence Prevention Act requirements.
