Location : West Harlem
Ratio / Caseload : Entire team 48 clients at any given time. 5-7 clients for team lead
Age : 18-25
Setting : Field / Onsite –1 / 3 Field - 2 / 3 Office
Schedule : Monday through Friday 9 : 00am to 5 : 00pm

Responsibilities
Case Management and Client Support
- Manage a small caseload of clients.
- Conduct assessments and develop individualized treatment plans.
- Facilitate client access to benefits, psychiatric care, medication management, rehabilitation, and other essential services.
- Connect clients with community-based resources, including health, mental health, and substance use services.
- Accompany clients to appointments when necessary.
- Communicate with clients’ families to provide updates on progress and address needs.
- Act as a liaison between clients and other service providers.
- Keep accurate and up-to-date client records in accordance with program policies.
- Visit clients in diverse settings, including residential facilities, public spaces, and other locations, while advocating on their behalf.
- Participate in an on-call rotation to provide support outside of standard working hours.
