Responsibilities:
- Responsible for medical records through preparation, compilation, analysis, maintenance and filing.
- Maintains a well-organized system of closed and open medical records so that records are available to interdisciplinary personnel as needed and other authorized activities.
- Upholds the confidentiality of medical records and protects the patient’s privacy rights
- Locates, pulls and charges our patient records and makes them available to the requesting provider/dept.
- Locates, pulls and charges outpatient records for audit and administrative purposes.
- Completes new patient charts and reassembles damaged charts or change of status charts
- Files medical record reports, forms and data into patient charts and files patient records into record storage areas.
- Ensures patient charts, paperwork, and reports are completed in an accurate and timely manner
- File all patients’ medical records and information
- Complete clerical duties including answering phones, responding to emails, and processing patient admission and discharge records
- Maintains hard copy files in an appropriate order
- Scan and attach appropriate documents to patient electronic records with accuracy
- Copy and/or printing of records for transfer
- Maintains tracking of records transfers and copying
- Obtain and document all pertinent information in the patient's charts, including EMR.
- Assists with training and orientation of new personnel.
- Maintains universal precautions utilized in all patient encounters.
- Reviews patient charts prior to releasing to requesting provider/dept for needed forms and reviews prior to refilling to assure completion. Replenishes forms as needed. Returns records to providers for completion when indicated.
- Other duties as assigned by management.
Qualifications:
- Three (3) years of experience in the field related to medical or health care, one (1) year experience of which is directly related to the medical records process (preferred but not required). Education or Experience may be substituted. Experience with Synergy EMR preferred (not required).
- Associate degree from an accredited university.
