Duties and Responsibilities:
Ability to consistently exercise discretion and appropriate judgement in the application of the advanced knowledge you have obtained in professional coursework, training, and experience for the benefit of the members with severe disabling mental illness (SDMI) or co- occurring disorder
Provide supports to all members of the psychosocial rehab program by facilitating groups, leading community integration outings, and individual supports as needed.
Provide education to teach the members about Mental Illness and appropriate medication to avoid complications and relapse, provide opportunities to learn and use independent living skills, and to enhanced social and interpersonal skills within a supportive and normalizing program structure and environment according to licensing and Medicaid
Other duties as assigned by the Program Team, Clinical Director and Executive
Knowledge, Skills and Abilities:
Knowledge of principles and techniques of Person-Centered Planning as applied to youth and adults: knowledge of principles and techniques of basic interviewing and assessment skills, working knowledge of Human Services System, role of Human Service Personnel, and relevant community resources; ability to form effective relationships with other internal and external team members; ability to communicate effectively both orally and in writing.
