Job Description
We are seeking an experienced primary care physician to fill a medical director role in one of our skilled nursing facilities. The ideal candidate will have 1 years of experience in primary care and a strong background in leadership. Responsibilities will include:
- Providing high-quality patient care through daily rounding and collaborating with facility staff to ensure effective treatment delivery.
- Managing chronic disease, diagnosing illnesses and medical conditions, and implementing plans of care.
- Collaborating with clinicians and facility leaders to address care coordination initiatives and network management issues.
- Representing TeamHealth externally, engaging with post-acute care communities, and establishing/maintaining relationships.
Required Skills and Qualifications
To be considered for this role, applicants must possess:
- An active and unrestricted Delaware license.
- An active and unrestricted Delaware DEA.
- At least 1 year of experience in primary care.
