The estimated salary for this role is $65,000 - $75,000 per annum, depending on experience. The schedule is Monday to Friday, 8am to 4:30pm.
A Master's Degree in Social Work and current State Licensure in Social Work are required. At least one year's experience in a healthcare or hospice setting is also necessary. The successful candidate will perform psychosocial assessments of patients and families/caregivers in a timely manner. They will ensure identified needs are addressed in the patient's plan of care. This includes counseling patients and family members/staff as needed, providing accurate documentation of patient services, and obtaining resources from community social, health, pharmaceutical, and welfare agencies to assist in meeting patient/family needs.
