Responsibilities:
- Assess patient physical, psychological, social, and spiritual factors that may impact patient and family needs and coordinate intervention by other members of the interdisciplinary team.
- Obtain data and evaluate necessary information to develop and implement individualized care plans.
- Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
Qualifications:
- Registered Nurse with at least one (1) year clinical practical nursing experience preferably in intensive care, rehabilitation, or medical surgical nursing (preferred).
- Home health experience preferred.
- Possess and maintain valid CPR certification.
- Must have reliable transportation, current driver's license, and appropriate automobile insurance.
