We are seeking a skilled and compassionate Social Worker to join our team at TN Quality Care Hospice in Waynesboro, Tennessee.
About the Role:
As a Social Worker, you will be responsible for providing psychosocial assessments of patients and their families/caregivers in a timely manner. You will also provide counseling and support to patients, families, and caregivers as needed, and assist with applications for benefits such as Supplemental Security Income and Medicaid.

- Perform psychosocial assessments of patients and their families/caregivers in a timely manner
- Provide counseling and support to patients, families, and caregivers as needed
- Assist with applications for benefits such as Supplemental Security Income and Medicaid
- Serve as liaison between patients and their families/caregivers and community agencies
Requirements:
- Bachelor's or Master's degree in Social Work (MSW) from an accredited program
- Tennessee state licensure in social work (LMSW or LCSW)
- Minimum one year of experience in healthcare or hospice setting
- Valid driver's license and proof of insurance
