Job Summary
A social worker is sought to provide psychosocial assessments and counseling services to patients and their families in a hospice setting.
Key Responsibilities
- Conduct timely psychosocial assessments of patients and families/caregivers to identify needs and develop plans of care.
- Counsel patients, families/caregivers, and staff on stress management and coping strategies.
- Provide accurate and timely documentation of patient services.
- Assist with applications for government benefits, including Supplemental Security Income and Medicaid.
- Serve as liaison between patients, families/caregivers, and community agencies.
Requirements
- Master's Degree in Social Work (MSW).
- Current State Licensure in Social Work.
- Minimum one year's experience in healthcare or hospice setting.
