Clinical Team Leader - ACT(Assertive Community Treatment) / SB292 Jail Diversion
Role Responsibilities
The position is responsible for oversight of the Assertive Community Treatment Team (ACT Team) for the Behavioral Healthcare division and the SB292 Jail Diversion program. The position provides direct supervision to ACT Team / SB292 Team members and works with other members not directly supervised (i.e. other BH Clinics, Benefits, Psychiatrists, the Crisis Center etc.) The position is responsible for ensuring fidelity to the SAMSHA evidenced based ACT Team model with particular attention to Texas requirements in the HHSC performance contract. This position monitors all referrals to the SB292 program and communicates with Fort Bend County that partners with Texana Center on this project. The position monitors ACT team services, monitors chart reviews to improve clinical care performance and documentation of those services, service delivery and ensures required weekly staffings are well documented. The position works with the most difficult to engage individuals and must have the passion to work with this population and positively lead a team. The position requires some direct communication with clients served in either program. The position requires the ability to communicate effectively verbally and in writing. The employee must have the ability to network and work collaboratively with others. The position reports directly to the Clinical Support Manager II and will perform other duties as assigned by the manager. Works a 40 hour work week, Monday through Friday with exceptions to meet program needs. Travel is required. Responsible and accountable for adherence to and promotion of the compliance program.
Salary
- 90% $77,850.00 annually
- 95% $82,175.00 annually
- 100% $86,500.00 annually
